Summer 2025 Inclusion Request Forms
RETURNING Summer Participants complete the form linked here. Please only complete this form if you requested inclusion support services or participated in a therapeutic recreation program through Burlington Parks and Recreation in the past year.
**NOTE: Completing the inclusion request form does NOT count as registering for the program. Registration must be completed FIRST at www.burlingtonrecreation.org**
